How a plan works.

Since 1 July 2025, a single GP chronic condition management plan has replaced GP Management Plans and Team Care Arrangements. Here is what that means for you.

Medicare sets these rules and they can change. Your GP or reception can explain how they apply to you.

              • What is a GP chronic condition management plan?

It is a written plan you and your GP put together for a condition that has lasted, or is likely to last, six months or more. It sets out your goals, the care you need and who will provide it. It replaced GP Management Plans and Team Care Arrangements on 1 July 2025.

              • Which conditions can a plan cover?

Plans can cover many long-term conditions, such as diabetes, asthma, COPD, heart conditions, arthritis and thyroid conditions. Your GP will talk with you about whether a plan suits your care.

              • Is my plan bulk billed?

Preparing and reviewing your plan with your GP is bulk billed for eligible patients with a valid Medicare card. Allied health providers, specialists and pathology providers set their own fees.

              • Can I get Medicare rebates for allied health?

If allied health care is part of your plan, your GP can refer you with a referral letter. Medicare rebates are available for up to five individual allied health visits each calendar year, or ten for Aboriginal and Torres Strait Islander patients. Providers set their own fees, so check with them first.

              • How often is my plan reviewed?

Your GP will talk with you about when to review it, and as your needs change. To keep using Medicare rebates for allied health, your plan needs to have been prepared or reviewed in the previous 18 months.

              • I had a GP Management Plan before July 2025. What happens now?

Allied health referrals written before 1 July 2025 stay valid until those visits have been used. Older plans can still be used for allied health until 30 June 2027. When your plan is next reviewed, your GP will move you to the new plan.

              • I am registered with MyMedicare. Does that change anything?

Yes. If you are registered with MyMedicare, your plan and its reviews need to be done through the practice you are registered with. Aberdeen Surgery has joined MyMedicare, so ask reception if you would like to register here.

              • Do I need a longer appointment?

Yes. Tell reception the visit is for a chronic condition plan or review when you book, so they can give you the right amount of time.

              • Can I keep seeing the same GP?

Where you can, book with your usual GP so your plan stays with someone who knows your history. You can choose your GP on HotDoc or ask reception.

              • What should I do if I get worse between visits?

Call reception on (02) 6543 8424 and tell them what has changed. When we are closed, call healthdirect on 1800 022 222 for advice. In an emergency, call 000.
